1. Go to the following URL: https://ssointernal.chfs.ky.gov .
2. Select Citizen or Business Partner and Select Create Account.
Please refer to the KOG Onboarding for CAN Check Requests if you do not have a Kentucky Online Gateway account.
3. Enter your registered e-mail address and password.
4. Select the letter “C” from the alphabet list and then select CAN Payment and Verification (Child Abuse and Neglect) from the application list.
5. The CAN Check Home screen will be displayed. Click the drop-down arrow next to Form and choose DPP-156.
