Before any athlete can participate in tryouts, practice and games, all paperwork and insurance fees are to be collected by the coach.

  (6 pages to be completed by a medical professional)

2022-2023 OCMS Required Athletic Packet
  13 pages including:   
          OCBE Permission to Participate
          OCBE Field Trip Permission form
          OCBE Alternate Transportation form
          ImPact Testing form *
          Academic Eligibility
          Parent Communication form
          Parent Code of Conduct

2022-2023 Waiver of Liability COVID 
This is DISTRICT-REQUIRED documentation that must be completed in addition to the physical and athletic packet

* NOTE: The ImPact Testing form is a new form regarding concussions. Although they are titled as high school forms, they are the correct forms for the middle school.
